The Community Health Education and Outreach Department of Phoenixville Hospital offers Diabetes Self-Management Education to persons with diabetes.  Diabetes Self-Management Care helps people start their diabetes self-care effectively when they are diagnosed.

 On an ongoing basis, participants in the Diabetes Self-Management Education program will be able to:

• Gain metabolic control
• Manage complications
• Improve their quality of life

Group Education Program:
This program consists of ten (10) hours of outpatient education covering specific content mandated by the American Diabetes Association. The program content includes nutrition, exercise, blood glucose monitoring, and medication.  The facilitators for the program are an RN, CDE and an RD.
